If all conditions that apply to sperm, ovum, or embryo recipients are satisfied, an infertile couple or a single woman shall send a dossier of application for in vitro fertilization to a medical examination and treatment establishment eligible to perform this technology, and comprising:

• An application for in vitro fertilization.
• The examination dossier determining the infertility of the woman or couple named in the application for in vitro fertilization.
